By the time the chart reaches you, the diagnosis, the causation determination, and the care plan are already set. Your chair is the ongoing one: you run the sessions, practice measurement-based care, draft the progress reports and authorization requests the psychologist co-signs, and steer the case toward discharge. The psychologist owns the evaluation and the causation; you own the treatment that proves the plan is working.
